Useful forms
We have produced a Risk Assessment form (PDF 28 KB) that follows the basic "Five Steps to Risk Assessment" method (which is ideal for most smaller Churches and Places of Worship).
Fire Risk Assessments need you to look at the sources of fire and the fuel that could be involved in fire. Our Fire Risk Assessment form (PDF 29 KB Revision 2) can be used to help you with Fire Risk Assessments.
This Safety Log Form (PDF 17 KB) can be used to record the inspections, tests and checks you do, and there is a section to write down any corrective actions you need to do. This download includes brief instructions and examples.
